Every Heijmans employee tries to do their job as best and as safely as possible. Even so, an incident will still take place from time to time. To enable us to learn from these incidents, we will focus on how to report them in this toolbox.

There is a certain relationship between accidents that come about. As you can see in the safety pyramid below, many unsafe actions or unsafe situations take place for every fatal/serious accident that occurs. By taking lessons from unsafe actions or unsafe situations, we can prevent fatal/serious accidents.
Among other things, reporting accidents enables us to:
1) Learn from incidents. Investigating incidents allows us to take suitable measures to prevent any repeats of similar or other incidents.
2) Gain insight into trends in safe and unsafe behaviour.
3) Highlight any sticking points to management. Management are often insufficiently aware of certain situations. As a result, these situations are not recognised as sticking points, and no action is taken to tackle them.
Report all unsafe situations, accidents, near misses and other unusual events: it's better to report ten situations too many than one situation too few. Make sure you report any situations immediately; do not wait until the following day.
Accidents, unsafe situations, environmental incidents, excavation damage and safe situations must all be reported via the GO! App.
Always use the 'Action list for emergencies and incidents' in serious situations. This list can be found under the 'In case of emergencies and accidents' tile in the GO! App. In serious situations, always contact the Safety Coordinator for your business unit by telephone.
Any inappropriate conduct, breaches of integrity and misconduct can be reported to a confidential advisor or the Compliance Officer at Heijmans.
When you report an accident or environmental incident in the GO! App, the following steps are taken:
1) The project manager, the manager of the profit centre and the Safety Coordinator receive an e-mail about the report. Together, they run through a unified accident management process — the so-called 1-3-14 process — to deal with the report. The person submitting the report and certain members of the management team will also receive an e-mail about the report.
2) Once the report has been closed off, the person who submitted the report will receive an e-mail letting them know. This e-mail will also provide more detail on the measures that have been taken.
When you report an unsafe situation or safe situation in the GO! App, the following steps are taken:
1) The project manager will receive an e-mail about the report and will be asked to deal with it. The person submitting the report and the Safety Coordinator will also receive an e-mail about the report.
2) Once the project manager has dealt with the report in the GO! App, the Safety Coordinator will be asked to assess how the report was dealt with. When the measures taken are adequate and the incident is not 'HIGH potential', the Safety Coordinator will close off the report.
3) Once the Safety Coordinator has closed off the report, the person who submitted the report will receive an e-mail letting them know. This e-mail will also provide more detail on the measures that have been taken.
When you report excavation damage in the GO! App, the following steps are taken:
1) A member of the finance team will receive an e-mail about the report and will be asked to deal with it. The project manager and certain members of the excavation damage prevention team will also receive an e-mail about the report.
When you fail to report an incident, you take away the opportunity for Heijmans to improve. In extreme cases, this may lead to an accident happening that could have been prevented.
